SSI Member Patients;
1. T.R. for all SGK patients (Bağ-Kur, SSK, Retirement Fund transferred) Identity document with written identification number (with photo for over 15 years old), address and telephone information statement is required.
2. Relatives and Veterans in need of T.C. They apply with their ID number. They need to be activated from SGK.
3. Members of foreign SSI health insurance, T.R. identification number, SSI health assistance document (the phone number and address information that can be reached during his stay in Turkey is mandatory.)
4. TAF personnel (military and civil servants) and their dependents, T.R. Identity document, non-commissioned officers and privates must apply with a dispatch document.
5.For students, T.C. identity card, address and telephone information statement is required.
Green Card Member Patients;
T.R. Identity document must be brought. (Correct statement of phone and address information is mandatory.)
Paid patients;
1. Identity document, cashier receipt, address and telephone information statement are required. (Correct statement of phone and address information is mandatory.)
Bank Employees;
1. Ziraat and Vakıfbank employees need a workplace referral, a report card and a photocopy of their identity card for spouses and children. (Correct statement of phone and address information is obligatory.)
2. For private bank employees, a copy of the workplace ID is required, for spouses and children a photocopy of the report card and identity card is required. (Correct statement of phone and address information is mandatory.)
Chamber and Exchange Employees;
A copy of the workplace referral and identity card is required for him, and a copy of the report card and identity card for the spouse and child. (Correct statement of phone and address information is mandatory.)
Private Insurance Company Employees;

Earthquake Victims and Disaster Victims;

Syrian refugees;
A residence permit, a document showing that they are registered in the police is sufficient.
Other Stateless refugees;
It is the number starting with 99 stated in the residence permit document. If it does not exist, it is registered with the virtual TC ID number.
